What is an ingrown toenail? An ingrown nail is defined as when the skin on one or both sides of a toenail develops over the nail edges or when the nail itself grows into the skin. The medical term for the condition is onychocryptosis. Although it most commonly occurs on the big toe, the lesser toes may also be affected. Ingrown toenails may be caused by a variety of factors. They can have genetic origins; the nail is just too large for the toe or may be curved, leading to the predisposition of the condition. Or they may be due to trauma to the area such as stubbing your toe, dropping a heavy object or having it stepped on. Improper clipping of toenails such as trimming too short leads to skin inflammation and shoe friction upon that skin further increases the pain. An impacted toenail can first be seen directly after improper grooming of toenails. The nail looks like it has been cut back too far and there is exposure of the pink tissue around the edge. Running often and hard leads to formation of ingrown toenails because the pressure on the toes from the shoes. Wearing tight shoes or shoes too small for the feet also lead to the condition because the toes have no room and the nails are becoming compressed. How do you know that you have an ingrown toenail? Besides causing discomfort and pain, there are other indications that lead to the diagnosis of an ingrown toenail. The signs of a toe with an ingrown toenail are redness, swelling, tenderness and rigidity. In some cases, there may be pus drainage from the area of the impacted toenail. In addition, the condition will not fix itself and the nail will not correctly grow out. The nail keeps growing into the tissue surrounding it and increasing the severity of the pain. When you take a toenail clipper to the nail, it will become harder to trim the nail back. The skin around the ingrown toenail is very sensitive to touch and even contact with socks and shoes will inflict pain! In addition, an ingrown toenail may lead to infection causing further inflammation and pain! Ingrown toenails are a common and uncomfortable foot condition that cause a lot of pain and discomfort. There are many reasons for ingrown toenail occurrence and some of those causes can be eliminated with education and hygiene. What are ways to prevent an ingrown toenail? The most frequent causes of ingrown toenails are improper toenail clipping and tight shoe wear. Toenails should never be longer than the tips of your toes, but do not cut them too short as to reveal the soft pink tissue underneath. This skin tissue is easily inflamed, causing pain. When attempting to cut toenails, make sure to use proper toenail clippers. A normal nail cutter has slightly curved cutting edges while a toenail cutter has edges that are straight across in order to reduce the likelihood of ingrown toenails. Observe the natural line of your toenails and when grooming toenails make sure to trim them straight across. Do not round off or cut the corners of your nails or pry the corners when cutting. Shoe wear Proper fitting shoes are essential for good foot health. Small, tight shoes not only compress the toes and toenails, they do not allow your feet to breathe, lead to foot discomfort, and may even affect the biomechanics of your walking. When shoe shopping, do it in the late afternoon or evening because you feet are at their largest size. In addition, make sure your foot size is measured every time before purchasing shoes to ensure an appropriate fit. What are treatments for an ingrown toenail? There are a variety of non invasive and invasive (surgical) treatments for an ingrown toenail: soak your foot in warm water, wear comfortable shoes with a sufficient room for your toes, consider wearing open toed shoes to prevent shoe material from touching the affected area, and take aspirin or ibuprofen for pain relief. As soon as you detect an ingrown toenail make an appointment with your podiatrist! An ingrown toenail may be complicated by an infection, so it is best to leave your problem in their hands. In the case of an infection, your doctor may prescribe oral antibiotics. Surgical intervention may be necessary to remove the offending part of the nail and prevent infection. A partial or complete nail avulsion is a procedure where the doctor injects anesthetic in your toe and then instruments are used to cut away the ingrown toenail portion without disturbing the nail bed. What we plan to do over the next few weeks here on the Injury Zone is some Q from runners that have reached out to us. The first topic we are going to discuss is running with pain and when runners should look to start addressing pain on their own, or with the guidance of a health care professional. Whether it before, during, or after a run, pain is something that all runners experience at some point in time. If you are in that very small percentage of runners that never sustain an injury, please contact me and let me know your secret! In all seriousness, running with pain is not fun and when runners start to experience discomfort, the first thing they do is it to find an answer. Let not kid ourselves, we all done this and frankly, with all the information available on the internet, it not a bad idea. Unfortunately, a lot of the information that is available can further cloud what you think is going on and leave you more confused! Pain is not something runners like to deal with, as it can prevent increasing mileage, frequency, and overall training regimens they follow. The key to how runners manage pain initially can be of great importance for long term recovery. Some questions I ask runners when they come to see me are 1) how often is the pain present 2) what activities other than running causes it, and 3) what is being done to help alleviate it. As a physical therapist, I always try and establish some baseline measurements with my clients, as it something we can monitor and measure as they progress through rehab. A pain scale that I use frequently with runners is from a fellow physical therapist Bruce Wilk, who runs his own practice down in Miami, Florida. He offers a very straight forward 1 5 pain scale based on when the pain is occurring and when it might be appropriate to start managing it. Stage 5 Pain that cripples you Wilk believes, and I agree with him, that if you are at stage 2 or further, that is the time to start self managing. Symptoms that don subside after activity and continue to persist need to be addressed to prevent further issues. If you notice that the pain is lingering into your daily routine, then it is probably in your best interest to seek help from a professional. The thing that sticks out to me most in this scale is stage 4, where most runners I talk to are taking some sort of medication before, during, or after their runs to help their symptoms. NSAID (non steroidal anti inflammatory drugs) such as Ibuprofen, Advil, or Aleve, are pain relieving medications that could potentially mask what your body trying to tell you. The more you rely on medication to get you through your runs, the more you run the risk of getting to the point of no return. Even if a runner looks at this scale and believes they are only at stage 1 or 2, if medication is used for pain management, the runner automatically is at stage 4. We are very up front with our runners and do not believe runners should be taking medication for pain management. Although it might help in the short term, it puts them at a high risk for potentially developing a serious injury. Our goals with rehab are to minimize the risk of injury and maximize what we know about it to help with recovery. Every person pain threshold is different, so this scale can help you determine when you need to start doing more to take care of your body. If you are, it might not be a bad idea to pay more attention to what your body is telling you and re assess what exactly you are dealing with. It was widely used for these purposes before the industrial revolution but its popularity then declined due to the availability of other textiles. It is hard to imagine but the strength from the hemp plant can be harnessed and used to strengthen buildings. Hempcrete, as it is known is made by mixing hemp hurds and lime. It is less brittle than concrete and therefore is more resistant to cracking reducing the need for expansion joints. Hemp has also been used in other composite materials for construction. The use of hemp in this way is really just in its infancy stage with it mostly being used for prototypes. Hemp can also be used to make the composite panels for cars. The hemp bast fibre is mixed with fibreglass for a strong material As well as it many other uses hemp oil, which is obtained from the fruit of the hemp plant, can be taken as a dietary supplement and has been shown to relieve the symptoms of eczema. It also has anti inflammatory properties and can be used for medical purposes. The hemp plant is also very useful for ecological reasons. It can be used to clean waste water removing impurities like sewage. It can remove excess phosphorus from chicken effluent or other chemicals. It has even been used to clear contaminants after the nuclear disaster at Chernobyl. The density and height that the hemp plant can grow to make it ideal for weed control. It minimizes the pool of weed seeds i n the soil and is particularly useful for controlling tough weeds, helping to reduce the use of herbicides. With its sustainability and many different uses, hemp could well be one of the crops of the future providing a partial solution to a number of environmental issues. There are however some issues surrounding the scale of hemp production which is partially limited by it status as a controlled crop in the US. With time, hopefully the production of hemp will become scalable allowing it to be used more extensively to make these many products.
